Not sure if you ever met someone with severe autism (Aspergers, by the way, is not longer used for diagnosis). I don't think I can convey it in a short comment, but if you meet someone who finds it simply impossible to understand other people's perspectives, you'll realize whether you call it a "medical" problem or simply a cognitive challenge, it is real.
